The Louisiana Bureau of EMS is responsible for the overall planning, coordination, licensing, and regulation of Louisiana’s EMS system. To fulfill this mission, the bureau utilizes this Information Management System (IMS).

  Initial applicants who completed initial EMS education outside of Louisiana must complete the required form for each state or territory where a license, certificate, or registration as an emergency medical services practitioner has been obtained:

  All applicants applying for either Initial EMT, AEMT, and Paramedic licensure or are applying for an EMT, AEMT or Paramedic license with an affirmative answer regarding criminal disclosure on the questionnaire must complete a request for a federal and state criminal background report utilizing the forms located on at: Click Here and submit to Louisiana State Police as noted on the forms.
